This cross-cultural validation study, conducted in Spain, confirms the reliability and construct validity of the scale, offering healthcare providers a valuable resource to proactively address medication adherence challenges. Bridging the Gap in Medication Adherence

Non-adherence to immunosuppressive medication is a critical challenge in the world of kidney transplantation, with up to one-third of recipients failing to adhere to their prescribed regimen. This phenomenon can lead to devastating consequences, including compromised graft function and even graft loss. Recognizing the urgent need to address this issue, a team of researchers embarked on a groundbreaking project to develop and validate the KATITA-25 questionnaire, a tool designed to identify patients predisposed to non-adherence even before they undergo transplantation.

Validating the Spanish Version of KATITA-25

The KATITA-25 questionnaire, originally developed in Brazilian Portuguese, has now undergone a rigorous cross-cultural validation process to create a Spanish version. This study, conducted in Spain, involved a systematic translation, back-translation, and cognitive interview process to ensure the scale’s linguistic and conceptual equivalence. The researchers then evaluated the reliability, criterion validity, and construct validity of the Spanish KATITA-25 in a diverse sample of kidney transplant candidates from four different healthcare centers in the Catalonia region.

Robust Validation Results

The results of the validation study were highly promising, aligning closely with the findings from the original Brazilian Portuguese version. The Spanish KATITA-25 demonstrated excellent internal consistency, with a Cronbach’s alpha of 0.83, and good test-retest reliability, with an intraclass correlation coefficient of 0.86. Confirmatory factor analysis further validated the scale’s multidimensional structure, confirming its ability to assess various psychological factors that influence medication adherence, such as forgetfulness, skepticism, and safety concerns.

Addressing the Gap in Spanish-Language Tools

The availability of the Spanish version of KATITA-25 fills a crucial gap in the clinical landscape, as there have been limited validated tools to assess medication adherence in kidney transplant patients who speak Spanish. Unlike retrospective scales that focus on past non-compliance, KATITA-25 takes a proactive approach by evaluating a patient’s predisposition to non-adherence even before the transplant process begins. This allows healthcare providers to tailor interventions and support systems to high-risk individuals, ultimately improving the chances of successful transplantation and long-term graft survival.

Enhancing Patient Care and Outcomes

The implementation of the Spanish KATITA-25 questionnaire in pre-transplant evaluations holds the promise of transforming the way healthcare professionals approach medication adherence challenges. By identifying patients with a higher propensity for non-adherence, clinicians can now develop targeted interventions, such as educational programs, motivational strategies, and close monitoring, to address the underlying psychological factors that influence medication-taking behaviors. This proactive approach has the potential to optimize patient care, reduce the risk of adverse outcomes, and ultimately enhance the success rates of kidney transplantation in the Spanish-speaking world.
